A roof repair job turned fatal for an 18-year-old Toronto worker, who fell 14 floors to his death. Ontario Ministry of Labour investigators are trying to determine what went wrong. While the victim was reportedly wearing a harness, and proper safety equipment was in place on the roof, questions remain regarding whether the worker was tied off. Police say the roof was icy and winds were strong when they arrived on the scene after the victim fell.
