OSHA began an investigation after four employees at a plant were hospitalized as a result of a fire that occurred when employees attempted to unclog a machine of carbon black powder and polyethylene pellets. A willful citation was issued for alleged failure to use lockout/tagout control procedures when employees attempted to clear jammed machinery.
