A dairy farmer died when he got caught in a conveyor delivering feed to his cows. Nobody witnessed the incident, but a co-owner of the farm said he was probably trying to lift the machine back onto the roller track. He had done this many times in the past without first locking out power. Lockout is an important safety procedure in any industry.
