A worker suffered fatal injuries when he fell through a skylight located on a roof undergoing maintenance. As a result, the employer was charged with four OHS violations that generally allege that it failed to provide a safe workplace; ensure workers and supervisors were made familiar with the hazards; ensure fall protection was used; and ensure that work areas were arranged to allow the safe movement of workers.
